Home Care Objectives

• Provide continuity of care from the hospital to the home
• Include the patient and physician in developing a plan of care to meet the patient’s needs
• Shorten hospital stays and prevent re-hospitalization
• When at all possible, speed up the patient’s recovery and return to regular activities of daily living
• Reduce the cost of illness
• Collaborate with other community agencies to facilitate continuing care

Eligibility

Individuals are eligible for home health services regardless of race, gender, religion, age, national origin or economic status.

Reimbursement

Payment for services may be covered by Medicare, Medicaid or private insurance based on program guidelines. Home health services also are offered on a private-pay basis. Financial arrangements can be made, if needed.

Referrals

Referrals may be made by physicians, patients, families, friends or community agencies. As a result of the referral, information may be provided or an evaluation visit may be made to determine eligibility. All home health services are directed by the patient’s physician.

Certification/Accreditation

Avera Home Health Services are certified for Medicare and Medicaid reimbursement.
