It is our goal to provide complete and accurate information in our online Provider Directory. Because changes may occur before the Directory is updated, it is the Member's responsibility to verify that the provider is in the network before an appointment is made or services provided. Also, note that providers are participating with Avera Health Plans (AHP) only at the clinic sites listed. If you receive services at a different clinic site, the payment will be at the out-of-network benefit. To optimize your benefits, always use a participating provider.
