Vision Services Rider
In addition to the benefits provided in our standard group benefit plans, Avera Health Plans offers employers the opportunity to purchase a routine vision benefit for their employees. Avera Health Plans contracts with Vision Service Plan (VSP), the nation’s largest provider of routine eye coverage. Nationwide, VSP provides access to more than 18,300 eye doctors. The benefits that are included are:
- One refraction examination per calendar year with a participating VSP provider
- 20 percent discount on certain lenses and frames purchased from the VSP provider during the 12-month period following the exam
- 15 percent discount on contact lens services, for the VSP providers professional services
Avera Health Plans members who have this benefit included in their plan can locate a VSP provider for services by referring to a VSP brochure, which is provided in their membership materials, via the internet at www.vsp.com or by contacting the Avera Health Plans Sales Department at (605) 322-4545 or toll-free at 1 (888) 322-2115, Monday – Friday, 8 a.m. - 5 p.m. CT.

Family Planning Rider
Avera Health Plans has contracted with an outside vendor, Unified Life Company, to provide network access, processing of claims, member education and payment to providers for family planning services. Family planning services are not mandated in South Dakota, however are mandated in Iowa and Minnesota. South Dakota groups may purchase an option to include these services. When the services are part of the purchased benefit, the subscriber receives the Unified Life information in their Avera Health Plans welcome packet.

Flexible Spending Accounts
Avera Health Plans offers employer groups the following programs that are authorized by the Internal Revenue Service (IRS). These plans include:
- Premium Only Plan-A Premium Only Plan is an employee benefit plan that allows you to pay your share of the premium dollars for certain group insurance benefits before taxes are calculated, thus lowering your taxable income. By lowering your taxable income, you will have more take-home pay.

Premium Only Plan, Flexible Spending Account and Dependent Care Reimbursement Account -A Flexible Spending Account allows you to pay for certain health care expenses with pre-tax dollars. A Flexible Spending Account may be used for costs such as deductibles, coinsurance, copayments, vision care, dental care, prescription drug expenses and over-the-counter (OTC) medication expenses funded by your pre-tax dollars. By using your pre-tax dollars, you will have more take-home pay. A Dependent Care Reimbursement Account allows you to pay for certain dependent care expenses with pre-tax dollars. A Dependent Care Reimbursement Account may be used for costs such as day care, home care or nursery care services for dependent children under age 13. In certain circumstances, you may use it when you’re providing support for a disabled spouse, child or parent who lives with you and is your dependent.
The charges for Flexible Spending Accounts include:
- Monthly administration fee per plan participant*
- Company monthly processing fee
- One time fee for Plan Document Printing*
*May be negotiable depending on size of the group
